Proof of NYS required immunizations must be on file in the Health Office before your child enters school. New students (also K) and grades 2 & 4 must produce proof of a NYS physical exam by 10/15 or within 30 days of entering school.
Your child may receive medications (both daily and as needed) if they are received in their original container and are accompanied by a medication administration form completed by yourself and your doctor. Forms are available in the Health Office or may be downloaded from the eboard link.
If your child cannot participate in PE or recess, please alert the Health Office with a note from yourself or your doctor. An additional note will be required to restart these activities.
No latex products are allowed anywhere in Osborn—this includes latex balloons and gloves. Snacks, if necessary should be healthy choices of whole fruits or vegetables. Please so not send any peanut products to school for lunch or snack. Many students have life-threatening allergies. Thank you for your cooperation.
The Health Office is a proud supporter of the Osborn Committee for Better School Health and is active in promoting wellness. We serve to increase student, faculty and family education toward integrating healthier choices and wellness into their lives. If you are interested in participating in such a committee, please contact the Health Office.

NUTRITION AND HEALTH
Osborn School PTO has recently formed a Committee for School Health to support and continue wellness programs at Osborn. The committee is open to all parents and will consist of parents, teachers, administrators and students. The first goal of the committee is to develop and distribute a survey to learn the needs of the school community. The results will be used to guide the committee goals.
Several health and nutritional policies were implemented last year and are reflected in the paragraphs below.
Lunch
Peanut products should NOT be brought to school. Several of our students have life-threatening allergies, and we hope that your cooperation will help insure the safety of these children. Osborn works proactively with food services to insure that the snack items on sale during the lunch periods coincide with their healthy snack list. The Food Services Director, John Rubbo, works closely with the School Nutrition Committee in an effort to provide even better choices for school lunch.
Snack
Snack is no longer an integral part of the day, except for kindergarten. Children may bring snacks if necessary, and a list of acceptable nutritious snacks including fresh and dried fruits and vegetables has been provided on the school website.
Kindergarten Snack
Kindergarten classes will have a daily nutritious snack and a drink from the water fountain during the morning. The snacks will be provided by the parents. A snack schedule and list of acceptable snacks will be sent home the first week of school. Only items on the list will be accepted. Classroom teachers will assign each family 2 weeks throughout the year to send in snack for the entire class. Snacks for the week should be brought to school on Monday morning.
